For International Patients Who Do Not Reside in Japan/ without Japan's National Health Insurance
If you wish to receive medical treatment for ongoing/chronic conditions:

If available, please bring any referral letters, medical reports, or any other document related to your condition and treatment if any. Please note that you will be required to pay 2,000 JPY +tax as specified under the “special medical care coverage act (保険外併用療養費)” if you visit our hospital without a referral letter (issued in Japan).
- All care for international patients without Japan’s health insurance will be charged at a rate of 10 JPY per 1 medical remuneration point*.
*Medical fees are calculated by points in Japan and they are taxable. - Payments must be paid in full at the end of each visit. (Except for after-hours consultation)
- If this is your first time to visit our hospital or it has been over a year since your last visit, you are required to pay the initial consultation fee.
- Please note that following fees may apply in addition to the total medical costs including the initial/subsequent consultation fees.
- Please note that you will be required to pay additional fees specified under the “special medical care coverage act (保険外併用療養費)” if you visit our hospital without a referral letter (issued in Japan).

❖ Please note that we do not accept direct appointments from overseas patients.
All the necessary information regarding your medical condition obtained by the medical travel assistance company will be reviewed by our physician to determine if we are able to accept your request or not. Once it is determined that we can accept your visit, we will proceed to arrange an appointment according to your preference. Once your appointment is confirmed, you may begin to arrange your trip.
❖ Please do not make your travel arrangement until receiving confirmation.
If you wish to receive medical treatment for acute conditions (a common cold, broken bones, etc.):
Appointment
If you or someone with you can speak Japanese, please call the following numbers to make an appointment in advance. (Available in Japanese Only) Appointments must be made by the day before your preferred appointment date.
Note:
If you are not able to speak Japanese or have limited understanding of the language, please bring an interpreter or someone who can assist you in communicating with our staff. If you would like us to arrange an interpreter for you (English and Chinese only), please make a request when you book an appointment. Please note that our interpretation services may not be available for your preferred day or time due to the limited number of interpreters.
Registration
Please arrive in the hospital and come to the reception desk at least 15 minutes before your appointment time.
What to Bring on The Day of Your Appointment
❖If you do not reside in Japan, please present a passport at the reception. The information listed on your passport may be necessary in case the assistance from the consulate or embassy of your country is required.
Payment
The medical travel assistance company of your choice may require you to pay the medical bills up-front. Please check with the medical travel assistance company carefully before receiving our medical care to avoid any trouble.
Fees and Charges
・Initial Administration Fee (only for the initial visit) 10,000 JPY+tax
・Interpretation Service Fee (if requested) 3,000JPY/hr+tax
※Interpretation service is available in English and Chinese only.
(Advance reservation required)
・Coordination Fee 5,000+tax
Private Health Insurance
Payments must be made in full at the end of your visit and you may submit a claim to your insurance company afterwards. If you wish to obtain medical documents* to be submitted to your insurance company, please make a request as soon as possible.
*Note: A designated form may be required for a medical certificate to be submitted to your insurance company. Please check with your insurance company in advance.
After-Hours Medical Care
All patients who have received after-hours medical care (after the cashier’s desk is closed) are required to pay a deposit in cash. The deposit will be reimbursed during the regular hours. (Additional charges will be incurred for after-hours consultations)

For International Patients Who Reside in Japan / with Japan’s National Health Insurance
If you are a mid- or long-term resident in Japan (more than 3 months), you are required to be enrolled in some form of public health insurance.
Fees and Charges
- All care for patients with Japan’s national health insurance will be charged at a rate of 10 JPY per 1 medical remuneration point*.
*Medical fees are calculated by points in Japan and they are taxable. - If you are insured, you are required to pay 10-30% of the total medical costs.
- Please note that if you are visiting our hospital for the first time or it has been over a year since your last visit, you are required to pay an initial consultation fee. In addition, if you are visiting our hospital without a referral letter (issued in Japan), you are required to pay 2,000 JPY +tax based on the “special or specified medical care coverage.

- Health Insurance Card Issue in Japan
❖If you don’t have your health insurance card with you at the time of your visit, you will be required to pay 100% of the medical costs. However, you may apply for reimbursement later.
- Photo ID (residence card, passport, driver's license, Individual Number Card (My Number Card ), etc.)
We may ask you to present a photo ID at the time of your first visit for reasons of medical safety such as preventing patient misidentification. Your cooperation will be much appreciated.
- Patient Registration Card
This is only necessary if you have already received medical services at any of the Koga Group Facilities.
- Medication records
Please bring medications you are taking now as well as your medication records.
- Other Documents
If available, please bring any referral letters, medical reports, or any other document related to your condition and treatment. Payment
- Once your consultation or treatment is completed, please return to the reception desk and hand the plastic folder to our staff to receive a number slip.
- When the number on your slip appears on the screen, please make a payment with the automated payment machine or at the cashier window (if you are paying with a credit card).
❖ Payment must be made at the end of each visit.
❖ If you have any questions regarding your medical costs, please feel free to ask the receptionist.
