About Outpatient Clinic
Appointments/Inquiries
If you wish to make an appointment, please call the number listed below (in Japanese only). Since the patients with appointments will be prioritized, it is recommended to book an appointment by the day before your preferred date (except Sundays and public holidays).
※Patients who require urgent care will be prioritized regardless of appointments.
※If you are not enrolled in Japan's National Health Insurance, please see "For International Patients" page.

1Reception
Please come to the reception desk located on the right after entering the building.
- What to Bring
- Health insurance card (including the Latter-Stage Elderly Healthcare Insurance Card and other certificates)
- Please bring the following if available
- A referral letter or other documents from your primary doctor
- Information sheet of your medications etc.
- What to Bring
- Patient ID Card (You may use the card issued at Koga Hospital 21, Shin Koga Clinic or Nobusuma Clinic)
- Health insurance card (including the Latter-Stage Elderly Healthcare Insurance Card and other certificates)
- Please bring the following if available
- A referral letter or other documents from your primary doctor
- Information sheet of your medications etc.
For Those Who Are Visiting Us for the First Time
Please take a red number ticket and wait.
If you are visiting our hospital without a referral letter, you are required to pay 5,000 JPY (+tax) in addition to the initial consultation fee based on the “special or specified medical care coverage.
For Those Who Are Re-visiting Us
Please take a red number ticket and wait If you have a referral letter or other documents, you wish to receive the secondary health examination or you do not have an appointment.
Please note that if it has been over a year since your last visit or you do not have a referral letter, you are required to pay 5,000 JPY (+tax) in addition to the initial consultation fee based on the “special or specified medical care coverage.
For Those Who Have an Appointment
If you are arriving at the hospital before 8:00am, please take a blue number ticket to be served accordingly from 8:00am.

2Tell Us about Your Condition (for the first-time patients only)
You will be asked about your condition by a nurse with medical questionnaires. A doctor will be assigned after your symptoms are checked.

3Wait in The Waiting Area
Please wait till your consultation starts.
Once your number is called, please enter the consultation room assigned. -
4Consultation/Testing/Next Appointment
You will have a consultation with a doctor and tests if necessary.
If you have any questions or concerns, please do not hesitate to ask. After the consultation, please receive an appointment sheet if you've made your next appointment and also receive a prescription if you are currently on medication.※If you've received a prescription, please go to a pharmacy within 4 days from the issue date to receive your medication.

5Cashier/Next Appointment
Please go to the cashier window 6 and receive a number slip for payment.
And, if necessary, we will arrange your next visit. -
6Payment
Your number will be displayed on the monitor once your bill is ready. If your number shows up, please use the automated payment machine to make a payment.
If you wish to pay with a credit card or debit card (some cards may not be accepted), please go to the cashier window 7.
